Combination therapies.

Each combination requires its own evidence

What is this method?

Combining two methods may alter progression control, but the benefit and risk must be assessed for the specific combination. The effectiveness of the methods alone does not prove the safety of their simultaneous use.

Mechanism and idea

The idea is to combine different stimuli that influence eye growth. This is the hypothesis tested in the study; not a guarantee of additive effect.

How has the evidence developed?

The most developed part of this layer includes atropine with orthokeratology. There is also increasing data regarding RLRL with spectacle designs and orthokeratology.

Key randomized trials

2026RCTSpain

Repeated low-level red-light therapy combined with orthokeratology for myopia control in Spain: a randomized controlled study

A Spanish RCT showed greater axis control when RLRL was added to orthokeratology than with orthokeratology alone.

Small, single-center and unblinded trial. This is a combination treatment study, not a European monotherapy RCT RLRL .

The review is not a substitute for checking a specific intervention. The publication date and query end date may vary.

2026Meta-analysis RCTGlobal

Efficacy and safety of low-concentration atropine combined with orthokeratology for children with myopia: a systematic review and meta-analysis of randomized controlled trials trials

A meta-analysis of RCTs showed additional inhibition of axis extension for low atropine with orthokeratology compared to orthokeratology alone.

Differences in observation times and protocols; collective benefit does not imply indication for every combination.

What do longer observations tell us?

The conclusions depend on the pair of interventions and the time of the study. We don't assume that a short-term advantage means the same difference in adulthood.

Safety

Monitoring must take into account both components, tolerance and possible contraindications of the device. The safety of concurrent RLRL and atropine cannot be derived from a study comparing two separate arms.

Limitations and open questions

DIMS + RLRL in the referenced work is a retrospective analysis; RLRL + orthokeratology in Spain is a small RCT. None of this work establishes the best pattern for every child.

  • Which children benefit further?
  • Does an increase in therapy load impair regularity?
  • How to sequence methods and when to limit treatment?

When is it considered?

The combination is considered individually, including: with an insufficient response to the current proceedings. The decision must be based on the evidence for this combination and documentation of both interventions.

What should be monitored?

  • Effect and tolerability of each ingredient
  • Refraction, axis length and method specific measurements
  • Regularity and family burden
  • Interactions and contraindications in current documentation

This is a framework for a conversation with a specialist, not an individual treatment plan.
