The Nowak Method 1.1: biometry, safety and clinical responsibility

A new edition of the expert position paper by Professor Michał Szymon Nowak is available. Version 1.1 expands the account of the evidence base and clarifies where the data end and clinical judgement begins.

The document Expert Position and Clinical Implementation Guidance for the Management of Myopia Progression in Polish Children and Adolescents has a new record in Zenodo: DOI 10.5281/zenodo.22698142 . The 1.1 release date is 7 September 2026. This overview is published 12 September 2026.

What has changed from 1.0?

A comparison of both documents shows three important developments. They do not change the principle that clinical eligibility assessment and therapeutic decisions rest with the doctor.

  • Regional references for the axial length of the eye. Chapter 6 of the 1.0 version indicated a gap in the reference data and the need for Polish percentile charts. The 1.1 version expands this chapter with regional grids for Europe and Australia. It separately describes the still existing need for data for the Polish population of children.
  • More precise separation of security data. Chapter 4.4 in the current edition separates test results, retinal imaging signals, and observation limitations. The absence of serious events in available studies is not presented as evidence of zero risk.
  • A clear line of evidence regarding termination of therapy. Section 4.5 indicates that the proposed gradual reduction of therapy frequency is based on expert opinion. The author distinguishes it from a clinical trial confirming the effectiveness of such a procedure and emphasizes the lack of a validated tapering protocol.

Basis of comparison: version 1.0, sections 4.3–4.4 and 6 and version 1.1, sections 4.4–4.5 and 6.1–6.4 .

Biometrics at the center of observation

The current document focuses on serial measurements of the axial length of the eye and the interpretation of the rate of change in the context of the child's age and development. Regional reference data provides a reference point; they do not replace the assessment of an individual patient. Chapter 6 does not introduce new clinical thresholds - it refers to the algorithm described in chapter 8.6.

The responsibility remains with the doctor

Nowak's method organizes clinical eligibility assessment, measurements, conducting therapy, monitoring and the decision to terminate it. The 1.1 version maintains the separation of responsibilities between the physician, the treatment center, the distributor and the device manufacturer. The device's emission parameters are specified and locked by the manufacturer; medical decisions are made by the doctor.

A document that can be verified

Version 1.1 is an expert position and an implementation framework. It is not a new clinical trial. The source document, its bibliography and the previous version can all be checked. The version number and a separate DOI identify exactly which content is being cited.
